Please find attached the “Apply for Refund” form for Levels competitions. This form should be used to apply for a refund due to injuries or illness.

It’s important to ensure that you have a valid medical certificate to upload when submitting the jot form. We are unable to process refund requests without the required medical documentation.

If you have already applied for a refund within the past two months, please reapply using the dedicated tab on the Team app. This tab is available at all times for your convenience.
